   Pre-printed Membership Application forms are available at our unicycling gatherings, and all unicyclists (or their parents) are expected to sign them.

The following is a low-quailty GIF image. If you want to print it, first save it to your hard drive then use image-editing software to change the resolution to 120 pixels-per-inch. Make sure all the fine print is readable and nothing gets chopped off.
